To obtain a transcript of your GED scores or a duplicate of your North Dakota GED High School Diploma - send a request in writing or fill out the attached form.
	Include in your written request:
	Your full name Your date of birth Your Social Security Number Your current home address Your telephone number The address where you would like the transcript sent (if other than your home address) The appropriate fee
	Duplicate GED Diplomas are $10 each. Official GED Transcripts are $2 each. Please send a money order. Do not send cash. Mail your request to: Department of Public Instruction CKEN-11 600 East Boulevard Avenue Bismarck ND 58505-0440 Fax , phone and e-mail requests are not accepted. Please allow 7 to 10 working days for processing.
